USER EXPERIENCE
CLIENT: Any hospice caregiver.
CHALLENGE: Outpatient visits spend too much time "documenting" info and not "providing care".
SOLUTION: A phone/tablet app that allows patients the ability to report condition prior to nurse visit.

STEP ONE:
User receives text message reminder 1 hour before scheduled appointments to self-assess.

STEP TWO:
Upon login, patient and/or caregiver answers 14 pre-determined questions. Questions ranked on "safe, moderate, danger" method. Users can also include customized questions and answers.

STEP THREE:
Once form submitted, patient's record is automatically updated.
Hospice worker can monitor patient's overall progress and also provide quality care during visits.
